Industrial Attachment Procedures

All first and second year students are to take note of the Industrial attachment procedures:
Step 1:
Students are to submit their Acceptance letters/Assurance Note from industry/organization to the Industrial Liaison Office.
Step 2
Students are to purchase their log books at the Bookshop (Administration block).
Step 3
Students’ Confidential Assessment Form; would be provided at the bookshop in addition to the log book.
Step 4
On assumption of duty, students will fill the Assumption of Duty Form Online at koforiduapoly.edu.gh within the first week.
Step 5
Lecturers/Supervisors will be sent to supervise all students on attachment at their various locations on a scheduled date.
